Forensic Community Liaison Nurse

Forensic Community & Liaison Nurse - Registered Nurse (RN)/ Registered Psychiatric Nurse (RPN)
Overview of Responsibilities
- Perform comprehensive initial intake and ongoing biopsychosocial assessments, including evaluations of mental status, physical health, and special needs. Develop and implement an Integrated Treatment Plan (ITP) based on clinical risk assessments and needs analysis.
- Facilitate community case coordination and advocacy for clients by maintaining communication with mental health teams, private practitioners, and social service agencies to ensure continuity of care and effective information sharing.
- Ensure accurate and thorough documentation of client information in compliance with regulatory standards, supporting treatment plans and clinical case planning.
- Deliver nursing services and therapeutic support, addressing immediate mental health needs, providing counseling, and assisting clients in accessing essential resources such as shelter, food, and financial assistance.
Qualifications and Skills
- Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
- Minimum of two years of relevant experience working with individuals facing severe psychiatric challenges.
- Valid driver's license with the ability to travel locally as needed.
Knowledge and Expertise
- Extensive understanding of nursing theory and forensic psychiatric nursing practices.
- Familiarity with professional standards and ethical guidelines set by the regulatory body.
- Knowledge of community resources and interdisciplinary collaboration in client care.
- Awareness of legislative frameworks and their implications for mental health services.
- Commitment to ongoing education regarding Indigenous rights and cultural safety.
