Nurse Coordinator for Discharge Planning in Pediatric Rehabilitation

As a vital member of the multidisciplinary team at PHSA, this leadership role focuses on the seamless coordination of discharges from the Acute Rehabilitation Unit (ARU) for pediatric patients and their families. The coordinator plays a key role in managing the intricate discharge process and collaborates closely with community resources and programs.

Key Responsibilities

  • Provides guidance and support to bedside nurses, healthcare team members, and families in the assessment, planning, facilitation, follow-up, and evaluation of pediatric patient discharges. This includes coordinating discharge preparation activities such as acquiring necessary equipment and nursing support, making referrals to appropriate outpatient and community services, and ensuring a smooth transition from hospital to home care.
  • Works alongside the interdisciplinary team to develop tailored discharge plans for individual patients, orders home supplies as needed, and identifies potential barriers to discharge for patients and their caregivers, striving to resolve any issues related to patient care during the transition.
  • Facilitates educational sessions for patients and caregivers, which includes creating teaching resources, identifying learning needs, and conducting instructional meetings.
  • Supports a family-centered care model by promoting education and teaching from admission through discharge, including the development of educational materials and mentoring bedside nurses.
  • Actively participates in interdisciplinary team meetings, coordinating with community care providers when necessary, utilizing phone conferences or TeleHealth as appropriate.

Qualifications

  • Graduation from an accredited School of Nursing with current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • A minimum of three years of recent clinical experience in a tertiary pediatric hospital or an equivalent combination of education, training, and experience.

Skills & Knowledge

  • Proven ability in developing nursing care and educational plans.
  • Comprehensive understanding of evidence-based nursing practices related to patient acuity and bed utilization.
  • Familiarity with various healthcare disciplines and their roles in patient care and discharge planning.
  • Awareness of critical perspectives and social justice frameworks, including Trauma-Informed Care.
  • Commitment to understanding legislative obligations and provincial commitments as outlined in foundational documents related to Indigenous rights and reconciliation.
  • As a valuable asset, knowledge of the socio-economic, political, and historical contexts affecting Indigenous Peoples, along with experience in addressing Indigenous-specific anti-racism and cultural safety.
