Prairie Mountain Health is seeking a highly skilled Registered Dietitian to join our team in promoting cardiovascular health and implementing the Manitoba Diabetes Strategy. As a key member of our health promotion team, you will collaborate with partners to develop and implement strategies to prevent diabetes and cardiovascular disease in our region.
ResponsibilitiesHealth Promotion and Community Development- Collaborate with partners to implement the Manitoba Diabetes Strategy and promote cardiovascular health.
- Assess and identify priority needs related to health promotion and prevention of diabetes and cardiovascular disease in the region.
- Lead the planning, implementation, and evaluation of health promotion strategies related to diabetes and cardiovascular disease.
- Contribute facilitation, communication, and resource skills to strengthen existing resources for prevention of diabetes and cardiovascular disease.
- Collaborate with partners to offer education for individuals, families, groups, communities, and health providers regarding prevention of disease, management of diabetes and cardiovascular disease, and avoidance of complications.
- Lead teaching and facilitation roles through individual assessment, planning, implementation, and evaluation of the nutrition care plan with clients and their significant others.
- Utilize current resources to provide accurate information and maximize client interest, participation, and learning.
- Lead an interdisciplinary Diabetes and Cardiovascular health care team to address education, risk reduction, diabetes and cardiovascular disease management, and prevention of complications.
- Incorporate best practices information, Community Health Needs Assessment data, and accreditation requirements in planning, implementation, and evaluation.
- Integrate current national guidelines into practice.
- Employ processes of Continuous Quality Improvement.
- Monitor and evaluate process and outcome indicators.
- Identify the required minimum data set.
- Collect data consistently, evaluate the data, and how the data is used.
- Participate in research as appropriate.
- Work with Community Partners to develop or enhance support networks.
- Advocate for and influence health policy for diabetes and cardiovascular health awareness, education, and environment supports.
- Apply communication theory and skills effectively.
- Nurture partnerships by sharing and using information, and evaluation.
- Maintain effective working relationships with clients, colleagues, supervisor, and partners.
- Facilitate/participate in collaborative decision-making with clients, colleagues, and other professionals.
- Respond appropriately to clients of diverse backgrounds.
- Report programmatically to Primary Health Care.
- Submit calendars, monthly reports, and statistics.
- Follow policy and collective agreement regarding hours of work and overtime.
- Perform administrative duties according to regional policy and the collective agreement.
- Document appropriately.
- Participate in ongoing community health needs assessment, strategic planning, program development, implementation, and evaluation.
