In this role, reporting to the Director of Care, the LPN will play a crucial part in enhancing the management and quality of nursing practices in alignment with the standards set forth by the College of Nurses of Ontario, Long Term Care Standards, and Southbridge Care Homes' guidelines.

Key Responsibilities:

  • Deliver resident care in accordance with established nursing standards, physician directives, and administrative policies. Monitor, document, and report vital signs, symptoms, and conditions of residents, initiating appropriate health interventions as necessary.
  • Conduct assessments of residents and develop, implement, review, and evaluate individualized care plans. Monitor health status, recognize changes in condition, and determine the need for medical referrals; process and execute physician orders.
  • Organize and oversee activities related to resident care, including restorative and palliative services as required. Interpret and apply established policies, procedures, and regulations.
  • Plan, direct, supervise, and assess the performance of nursing and personal care staff assigned to the unit. Facilitate the orientation and training of new personnel and participate in ongoing education initiatives. Evaluate staff performance and address any issues with the Director of Care.

Qualifications:

  • Valid certificate of competence from the College of Nurses of Ontario is required.
  • Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
  • In-depth knowledge of nursing and healthcare practices, including infection control protocols.
  • Strong interpersonal skills and a demonstrated commitment to working with the senior population.
  • Excellent communication abilities and the capacity to work both independently and collaboratively are essential.
