The Clinical Nurse Specialist in Home Care Services, Personal Care Home, and/or Transitional Care utilizes the nursing framework to assist clients/residents/patients in reaching and sustaining their highest health potential through a patient-centered methodology. This role encompasses working in various environments including acute care, transitional settings, Personal Care Homes (PCH), primary health care, public health initiatives, wellness programs, and/or home care services.

Employment Requirements:
