Key Responsibilities- Program Development: Collaborate with our team to design and implement rehabilitation programs that cater to the unique needs of our clients.
- Client Assessment: Conduct thorough assessments of clients' skills, strengths, and development needs to create personalized rehabilitation plans.
- Intervention and Support: Provide crisis intervention, suicide prevention, and substance abuse treatment programs to support clients in their recovery.
- Supervision and Coordination: Supervise clients living in group homes and half-way houses, ensuring their safety and well-being.
- Community Liaison: Establish and maintain relationships with other social services agencies and healthcare providers to ensure comprehensive support for our clients.
- Program Evaluation: Monitor and evaluate the effectiveness of our rehabilitation programs, making recommendations for improvement.
- Staff Supervision: Supervise and support office and volunteer staff to ensure the smooth operation of our services.
- Referral and Linkage: Refer clients to relevant agencies providing financial assistance, legal aid, housing, medical treatment, and other essential services.
- Program Planning: Develop and implement programs of assistance for clients, addressing their unique needs and circumstances.
- Case Management: Conduct individual, family, or group interviews to assess clients' situations and problems, determining the types of services required.
- Eligibility Determination: Assess the eligibility of individuals applying for benefits, ensuring fairness and accuracy in our decision-making process.
- Case History and Assessment: Prepare case histories and assess problems to inform our rehabilitation plans and services.
- Program Delivery: Plan and organize the delivery of social and community service programs, ensuring they meet the needs of our clients.
- Education: Secondary (high) school graduation certificate
- Experience: 1 year to less than 2 years
- Work Language: English
- Work Hours: 35 hours per week
