Job Summary:
The Nurse Practitioner - Cardiac Health Specialist will play a key role in the Cardiac Health Program at Trillium Health Partners. This advanced practice nurse will be responsible for providing high-quality patient care, leading clinical teams, and contributing to the development of evidence-based practice standards.
Key Responsibilities:
- Provide advanced patient care assessments and coordinate care for patients with complex co-morbidities.
- Order diagnostic tests, manage pain and symptoms, and educate patients in an in-patient setting.
- Engage in clinical mentoring and academic activities through clinical appointments with academic institutions.
- Collaborate with physicians, nurses, and inter-professional team members to provide quality care and achieve quality clinical outcomes.
- Develop policy and practice standards to support a patient-centered approach across the organization.
- Evaluate quality of care and patient outcomes within the Cardiac Health population.
- Identify quality initiatives to strengthen and develop nursing professional practice within Cardiac Health.
- Provide leadership and mentorship to team members through formal and informal education initiatives.
Requirements:
- Master's level nurse practitioner preparation required.
- Current registration in the Extended Class with the College of Nurses of Ontario required.
- 3-5 years of clinical experience in acute cardiac adult patient care required; cardiac surgery experience preferred.
- Minimum of one year of clinical experience in adult cardiac surgery patient management required.
- Previous Nurse Practitioner experience in Cardiac Health required.
- Strong problem-solving and negotiation skills.
- Ability to practice independently and interdependently.
- Demonstrated leadership to advance the practice of nursing and inter-professional practice.
- Excellent communication skills and ability to develop positive relationships with patients, families, physicians, and inter-professional team members.
- Demonstrated use of theory and research to enable evidence-based practice.
- Experience with research methodology, program planning, and evaluation.
- Current Basic Cardiac Life Support certification.
- Current Advance Cardiac Life Support certification.
