Key Responsibilities- Oversee the development, coordination, implementation, and assessment of the PATH (Priority Assistance to Transfer Home) program.
- Engage in discharge planning for clients in collaboration with hospitals and community support services.
- Evaluate and approve referrals, conduct intakes, and determine appropriate care coordination.
- Manage service requests and transportation scheduling, ensuring timely communication with hospitals.
- Ensure adherence to safety protocols and policies established by the agency and its partners.
- Conduct follow-ups with PATH clients within 48 hours post-service delivery to assess satisfaction and identify further service needs.
- Assist in budget preparation and monitoring specific to the program, and contribute to statistical reporting and progress documentation.
- Collaborate with hospitals and participating agencies to establish necessary qualitative and quantitative evaluation metrics for the program.
- Actively participate in the identification, development, and implementation of new or revised services and initiatives.
- A Bachelor's degree or a combination of relevant experience and education (1-3 years).
- Fluency in English is essential; proficiency in French is advantageous.
- Possession of a valid driver's license and access to a reliable vehicle for community travel.
- Proficient in Microsoft Office Suite (Word, Outlook, Excel, PowerPoint).
- Demonstrated experience in program management and development.
- Strong communication and presentation skills, along with leadership experience in client and volunteer engagement.
- Experience with administrative functions, including reporting and scheduling.
- Standard First Aid and CPR-C certification is a plus.
- Familiarity with community support services is beneficial.
- Most tasks will be conducted in a comfortable office setting, with occasional travel required within the designated area.
- Eligibility to work in Canada is necessary; candidates should verify their work eligibility status.
- Successful candidates will undergo a pre-employment screening process, including an Enhanced Police Information Check (E-PIC).
- Full vaccination against COVID-19 is required for this role.
Compensation Range: $43,746 - $45,000
