Registered Practical Nurse

We are seeking a Registered Practical Nurse (RPN) to join our team at Maxville Manor, a 122-bed long-term care home located in the village of Maxville, Ontario. The successful candidate will work in a full-time night rotation and have the opportunity to work part-time shifts.
Responsibilities- Assist the Unit Manager in completing and ensuring resident assessments are accurate and up-to-date
- Plan, coordinate, manage, and evaluate resident care plans in collaboration with the resident, substitute decision maker, physician, nursing team, and contracted service providers
- Process and follow orders from the Medical Director and/or Nurse Practitioner
- Assess and report resident response to care in a timely manner
- Prepare for scheduled care conferences and participate as applicable
- Respond to resident and family concerns and ensure appropriate action is taken
- Administer medications and treatments safely and accurately, and document response to treatment
- Provide education and support to residents and families, maintaining an open and knowledgeable rapport
- Respond to and manage significant changes in resident condition and medical emergencies, providing emergency first aid
- Supervise and direct Personal Support Workers (PSWs) while promoting a multidisciplinary team approach to care and service
- Current registration as a Registered Practical Nurse in the province of Ontario
- Experience working in a long-term care environment and with unregulated care providers preferred
- Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills
- Demonstrated knowledge and skills in current geriatric nursing care and standards preferred
- Computer proficiency in PointClickCare and experience with MDS-RAI or other electronic documentation systems required
