Job Title: Clinician, Geriatric Community Mental HealthAbout the Role

We are seeking a skilled Clinician to join our team in providing assessment, treatment, consultation, and follow-up services to mentally ill elderly clients of special complexity and difficulty, and their caregivers.

Key Responsibilities
  1. Provide clinical services to elderly clients and their caregivers, including comprehensive psychosocial assessments, treatment planning, and interventions.
  2. Collaborate with other team members to provide liaison with relevant community agencies and participate in the provision of community education in matters related to older adults with age-related mental health issues.
  3. Collect collateral information for team assessments related to competency (personal and financial).
  4. Provide follow-up, ongoing monitoring of complex psychosocial situations, and referral services where required.
  5. Participate as a member of the interdisciplinary team by coordinating work and cooperating with members of the team.
  6. Maintain clinical and statistical documentation according to established policies and procedures.
  7. Provide consultation, support, and informal education to families and other informal caregivers of the clients.
  8. Facilitate client and caregiver utilization of appropriate community services by keeping knowledgeable about other services and liaising with them.
  9. Provide consultation and develops care and treatment plans for shared clients in cooperation with care facility staff, physicians, Home Health, Specialized Seniors Clinics, and other appropriate partners.
  10. Act as an advocate on behalf of clients and their families.
  11. Maintain records and statistics, and prepares written reports according to policies and procedures.
  12. Provide crisis intervention.
  13. Provide ongoing liaison with assigned facilities/agencies in the community on a regular scheduled basis.
  14. Provide consultation and advice to colleagues and other teams.
  15. Interprets the role of geriatric teams to community groups and agencies.
  16. Participate in Mental Health and Substance Use Services training and planning activities as required.
  17. Supervises students as caseload permits.
  18. Provides consultation and education to referral agencies, care facilities, and caregivers towards improved care of the elderly with mental illness and represents the team on local committees and advisory boards as required.
  19. Performs other related duties as assigned.
Qualifications

We are looking for a candidate with a Master's degree in Social Work, Clinical Counselling, or other relevant health-related discipline, and one year's recent, related experience with Geriatric psychiatry clients in a recognized psychiatric and/or community mental health setting or an equivalent combination of education, training, and experience.

Eligible for registration with relevant professional association. Valid Class V BC Driver's Licence and possession of a personal vehicle.
