Community Health Nurse

Job Title: Part-Time Registered Practical Nurse (RPN) or Registered Nurse (RN) - Visiting Nursing in Mississauga
We are seeking a skilled and compassionate Registered Nurse (RN) and/or Registered Practical Nurse (RPN) to provide high-quality care to clients in the community. As a member of our team, you will have the opportunity to work collaboratively with a care team to deliver services to a roster of clients, providing comprehensive assessments, developing care plans, and implementing and evaluating care based on best evidence and practices.
About the Role:
- Provide client-centered care, working independently and as part of a team to deliver services to clients in their homes
- Develop and implement care plans that meet the unique needs of each client, using evidence-based practices and best available research
- Support clients through care transitions by connecting them with community resources and services

Requirements:
- Current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
- Gerontology competencies preferred
- Ability to be flexible and adaptable with excellent organizational skills
- Ability to take initiative and adapt to rapidly changing demands
- Excellent interpersonal, communication (oral and written) and critical thinking skills
- Demonstrated skills working independently and in a team environment
- A demonstrated appreciation for Quality Improvement processes
- A current CPR certificate is required
- A valid driver's license and vehicle (travel required)
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
- A willingness to attain required Saint Elizabeth certifications
- Availability to work days, evenings, nights and every other weekend
- Strong computer skills in MS Word, MS Excel, Email, etc.
