In alignment with Northern Health's commitment to an integrated system of services centered around Primary Care, the Primary Care Social Worker collaborates as part of an interprofessional team. This role emphasizes best practices to deliver thorough assessments, care planning, and interventions tailored to the needs of clients and their families.

Working in partnership with other healthcare professionals, the Social Worker provides direct psychosocial support, counseling, group facilitation, system navigation, advocacy, education, and connections to relevant specialized or community resources, all while promoting client self-management.

The Social Worker is tasked with delivering social work services aimed at enhancing the health and well-being of individuals, families, and communities.


Work Schedule: Monday to Friday, Day Shifts - 08:00 to 16:00

New Compensation :
As of April 1, 2024, the new compensation rate is: $52.81/hour

Required Qualifications

  • Bachelor's Degree in Social Work from an accredited institution.
  • Two years of recent relevant experience in a healthcare setting, or a combination of related education, training, and experience.
  • Current registration with the British Columbia College of Social Workers.
  • Valid BC driver's license.
