As a Registered Nurse at Halton Healthcare, you will deliver specialized patient care within a dynamic transitional care unit. You will play a vital role in providing compassionate support to both patients and their families. Your responsibilities will include coordinating care that emphasizes the patient's journey through the healthcare continuum, educating patients and families, and addressing factors that enhance customer satisfaction.
In this position, you will be empowered to deliver comprehensive nursing care within your professional scope. This includes administering medications and procedures in accordance with established protocols, conducting health assessments, documenting patient observations and changes, and collaborating with healthcare team members to revise individualized care plans aimed at achieving positive patient outcomes. Additionally, you may be required to take on charge responsibilities and offer support to your colleagues.
Required Qualifications
- Valid Registration in good standing with the College of Nurses of Ontario
- Current BCLS certification at the time of employment
- Proven experience in managing patients with tracheostomies, enteral/parenteral feeds, CADD pumps, PICC lines, IVs, and wound management systems
- Completion of a physical health assessment course
- Minimum one year of nursing experience in a Long Term Care environment
- Exceptional verbal and written communication skills
- Demonstrated compassion, patience, and empathy in patient interactions
- Proven leadership capabilities
- Ability to prioritize and manage multiple tasks based on patient needs
- Capacity to work collaboratively within a team as well as independently when necessary
- Proficiency in computer skills and CCRS data entry is an asset
- Commitment to ongoing professional development
- Strong interpersonal, organizational, and time management skills
- Must be able to fulfill the physical demands of the role
- Satisfactory employment and attendance history
Additional Information
Work Schedule : Rotating 12-hour shifts, Monday to Sunday (subject to operational requirements)
Compensation : Competitive hourly wage
