Qualifications and Requirements

**Education and Training:**

We are seeking a highly qualified individual with a Bachelor's Degree in Social Work from an accredited university. Additionally, the successful candidate must be currently registered with the British Columbia College of Social Workers.

**Experience:**

The ideal candidate will have a minimum of two years' recent experience in a healthcare setting, working with medically complex clients who have concurrent disorders. Alternatively, the candidate may possess a combination of education, training, and experience that includes comprehensive knowledge of chronic health challenges, geriatrics, community health principles, human development, behavior, and family/group dynamics.

  • **Counseling Skills:**
  • • Demonstrated ability to provide crisis, loss, and grief counseling.
  • • Demonstrated ability to work with patients in a flexible and adaptable manner.
  • **Relationship Building:**
  • • Demonstrated ability to build effective relationships with primary care providers, other disciplines, and community partners.
  • **Technical Skills:**
  • • Basic computer skills.
  • **Knowledge and Expertise:**
  • • Comprehensive knowledge of primary health care principles, community development concepts, and the strategic direction of the Vancouver Island Health Authority related to primary health care and chronic disease management practices.
  • **Facilitation and Conflict Resolution:**
  • • Working knowledge of facilitation, mediation, and conflict resolution skills and techniques.
  • **Analytical and Problem-Solving Skills:**
  • • Demonstrated analytical and problem-solving skills.
  • **Case Management and Clinical Support:**
  • • Demonstrated case management, clinical support, and consultation skills.
  • **Prioritization and Time Management:**
  • • Demonstrated ability to prioritize, work independently, and use a cooperative, proactive, and facilitative approach to patient care.
  • **Communication and Interpersonal Skills:**
  • • Ability to communicate effectively with the team, primary care providers, patients, their families, and caregivers.
  • **Clinical Resource and Education:**
  • • Ability to act as a clinical resource and educator, utilizing the principles of adult learning.
  • **Workload Management:**
  • • Ability to prioritize and organize workload.
