Job Summary

We are seeking a highly skilled Client Services Assistant to join our Home Health Team in New Westminster, B.C. This is a Casual opportunity that requires a strong ability to work independently and in cooperation with others.

Key Responsibilities
  • Coordinate financial, administrative, and contract aspects of client services and care management under the direction of the Manager, Clinical Services or assigned clinical staff.
  • Participate in care planning with clients by obtaining required client information from various sources and documenting client needs and wants.
  • Support long-term clients in accessing community or community-related services in accordance with established care plans.
  • Research community resources and identify information to assist in the development and implementation of client services and care management plans.
  • Organize individual and group client service meetings and schedule community appointments for clinical staff as required.
Requirements
  • Diploma in a social or health services field of study such as Community Social Service and two (2) years' recent related experience working with seniors or adults with physical and/or cognitive disabilities.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
  • Ability to work independently and in cooperation with others, communicate effectively, and analyze and resolve problems.
