About the Assertive Community Treatment (ACT) Program
The ACT program at UHN provides comprehensive support to adults living with severe and persistent mental illness. Our multidisciplinary team, including psychiatrists, nurses, social workers, and other professionals, works collaboratively to empower clients, integrate them into their communities, and enhance their overall well-being.
Position Summary
As a Community Mental Health Support Specialist, you will play a vital role in delivering person-centered care to individuals within the ACT program. You will build strong therapeutic relationships, collaborate with team members, and provide practical assistance to support clients in achieving their goals.
Key Responsibilities
- Develop and maintain positive therapeutic relationships with clients.
- Collaborate effectively with the multidisciplinary ACT team.
- Facilitate appropriate referrals to community resources and services.
- Provide practical support and assistance to clients in various areas of their lives.
- Participate actively in clinical rounds and case discussions.
Qualifications
- Bachelor's degree in Behavioral Science or a related field.
- Registration as a Social Service Worker.
- Experience working with individuals experiencing mental health challenges.
- Knowledge of psychosocial rehabilitation principles and practices.
- Strong time management, problem-solving, and communication skills.
- Experience in crisis intervention and de-escalation techniques.
- Familiarity with mental health policies and legislation.
- Proficiency in computer applications.
