Key Responsibilities:

The Social Worker is tasked with conducting thorough assessments, developing care plans, and coordinating treatment for assigned clients and their families.

In collaboration with primary care providers and the interdisciplinary team, the Social Worker delivers direct psychosocial support, counseling, group interventions, system navigation, advocacy, education, and connections to specialized care resources, all while promoting client self-management.

The Social Worker is responsible for enhancing the health and social well-being of the target population through effective service delivery.


Additional Duties Include:

  • Delivering direct social work services and care coordination for individuals, families, and groups.
  • Contributing a social work perspective to team practices; engaging in planning and program development; and offering operational, policy, and ethical practice recommendations through reporting and consultation.
  • Maintaining comprehensive client records, including assessments, treatment plans, progress notes, and reports, in line with established standards and regulatory requirements.
  • Serving as a resource for clients and their families by providing information on community resources, facilitating referrals, and aiding in the development of community support systems.
Work Schedule:

Workdays are Monday to Friday, from 08:30 to 16:30, totaling 37.5 hours per week.
