Social Services Coordinator

Job Summary
The Social Worker will provide assessment, treatment planning, counseling, service navigation, and case management services to families and individuals who are eligible for services. The successful candidate will work collaboratively with clients and their families to identify and address psychosocial needs, provide education and support, and facilitate access to resources and services.
Key Responsibilities
- Conduct initial assessments to determine client needs and develop treatment plans in collaboration with clients and their families.
- Provide counseling and therapy services to individuals and families, using evidence-based models and approaches.
- Facilitate access to community resources and services, including housing, financial, and vocational support.
- Collaborate with healthcare teams to provide comprehensive care and support to clients.
- Document client progress and outcomes in accordance with professional standards and organizational policies.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
- Master's degree in Social Work or a related field.
- Registration with the relevant professional regulatory body.
- Experience working with vulnerable populations, including children, youth, and families.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Working Conditions
The Social Worker will work in a fast-paced, dynamic environment, providing services to clients in a variety of settings, including community health centers, hospitals, and client homes. The successful candidate will be required to work flexible hours, including evenings and weekends, as needed.
