Job Title: Community Health Registered Nurse

We are seeking a skilled and compassionate Community Health Registered Nurse to join our Primary Care team at Flemingdon Health Centre. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality primary health care services to our clients.

Job Summary:

The Community Health Registered Nurse will work collaboratively with our team to provide comprehensive primary care services, including health promotion, education, and disease prevention. You will be responsible for assessing clients' needs, developing and implementing care plans, and providing support and guidance to clients and their families.

Responsibilities:
  • Provide comprehensive primary care services, including health promotion, education, and disease prevention
  • Assess clients' needs and develop and implement care plans
  • Work collaboratively with our interdisciplinary team to provide support and guidance to clients and their families
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
  • Develop and maintain working knowledge of community services and resources
Requirements:
  • Registered Nurse registered in good standing with the College of Nurses of Ontario
  • Bachelor's Degree in nursing
  • Minimum of three years' experience as a registered nurse within a community health centre setting
  • Strong assessment and communication skills
  • Ability to work collaboratively as part of an interdisciplinary team
