Nurse Practitioner

Vancouver Coastal Health is seeking a skilled Nurse Practitioner to join our team in the CHA 1 Home Health program. As a key member of our interdisciplinary team, you will provide semi-longitudinal care to vulnerable home-bound and frail patients, working closely with the Personalized Support & Stabilization (PSS) team to ensure comprehensive care.
Key Responsibilities- Conduct comprehensive assessments of patients and clients, including diagnosing diseases, disorders, and conditions.
- Develop and implement treatment plans, including health care management, therapeutic interventions, and prescribing medications as needed.
- Provide professional guidance to other healthcare professionals and participate as a member of the interdisciplinary health team.
- Collaborate with clients and other healthcare professionals to identify trends and patterns that impact client care, and develop strategies to improve health outcomes.
- Participate in peer, program, and self-review to evaluate the effectiveness of care and services.
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM) plus 4 years of recent, relevant clinical nursing experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE).
- Ability to perform the full scope of NP duties and responsibilities.
- Ability to apply knowledge of developmental stages, behavioral sciences, pathophysiology, psychopathology, epidemiology, and evidence-based practice to clinical diagnosis and patient management.
