Long-Term Care Registered Practical Nurse (RPN)
At Southbridge Care Homes, we are committed to delivering exceptional care to our residents in long-term care settings. We are seeking a dedicated RPN who is enthusiastic about enhancing the quality of life for our residents through compassionate and professional nursing practices.
Key Responsibilities:
- Oversee and execute best practices within the Resident Assessment Instrument-Minimum Data Set (RAI-MDS) framework.
- Collaborate with an interdisciplinary team to ensure comprehensive resident care.
- Facilitate educational sessions and training regarding RAI MDS protocols.
- Maintain high standards of resident care by adhering to established protocols.
- Monitor, document, and report vital signs and health indicators.
- Organize and lead activities related to resident care management.
- Support the administration and supervision of nursing personnel.
Qualifications:
- A minimum of 3-5 years of experience in Long-Term Care settings.
- Current registration with the College of Nurses of Ontario (CNO) in good standing.
- Proficient knowledge in nursing rehabilitation practices and collaboration with physiotherapy.
- Strong analytical and assessment capabilities.
- Experience in leading training initiatives for adult learners.
- Familiarity with the InterRAI suite of assessment tools and patient classification systems.
- Excellent communication skills to effectively interact with residents and team members.
