Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care Clinic in Chilliwack, BC. As a key member of our multidisciplinary team, you will provide comprehensive, patient-centered care to individuals across the lifespan, including chronic disease management.
Responsibilities- Assess and diagnose patients' health needs, developing and implementing treatment plans in collaboration with other healthcare professionals.
- Prescribe medications and order diagnostic tests as necessary, ensuring safe and effective patient care.
- Provide education and support to patients and families, promoting health literacy and self-management skills.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure seamless transitions between services.
- Participate in quality improvement initiatives, contributing to the development of policies and procedures that enhance patient care and safety.
To be successful in this role, you will require:
- Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
- Recent relevant clinical nursing experience, with a strong focus on primary care and community health.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong critical thinking and problem-solving skills, with the ability to analyze complex patient data and develop effective treatment plans.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with ease.
