Job Summary

We are seeking a skilled Social Worker to join our Community Wellness team. As a Social Worker, you will provide a range of client care services to ensure client engagement and reduce barriers to care. This includes extended community outreach, health education, case management, treatment support, counseling, group facilitation, community programming, and advocacy for clients with acute and complex care needs.

Key Responsibilities
  • Provide client care services to individuals, promoting wellness, disease prevention, and chronic disease management
  • Develop and implement community outreach programs to engage clients and reduce barriers to care
  • Collaborate with interdisciplinary teams to provide comprehensive care services
  • Facilitate group sessions and community programming to promote health and wellness
  • Advocate for clients with acute and complex care needs
Requirements
  • Bachelor's Degree in Social Work or related field
  • Registration with the Manitoba College of Social Workers
  • Minimum 5 years of experience working as a Social Worker
  • Experience working in addictions, mental health, crisis intervention, and cognitive behavioral therapy
  • Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
Preferred Qualifications
  • Experience working in an inner-city community setting
  • Knowledge and experience working with students and/or volunteers
  • Links/relationships with community resources
