Job Description
Overview
The Care Management Leader is responsible for ensuring the delivery of high-quality patient care in alignment with the standards set forth by the British Columbia College of Nurses and Midwives (BCCNM). This role emphasizes a client and family-centered approach, focusing on effective care coordination and discharge planning from the moment of patient admission.
Key Responsibilities
- Collaborate with an interprofessional team to facilitate timely and sustainable discharge plans.
- Conduct thorough nursing assessments and guide the team on the necessary interventions and resources to achieve optimal patient outcomes.
- Develop and implement policies and procedures that enhance care management and discharge processes.
- Participate in various committees and educational initiatives to promote continuous improvement in care delivery.
Qualifications
- Must possess a current practicing registration as a Registered Nurse with the BCCNM.
- A minimum of three years of relevant experience, including at least one year in community care and discharge planning.
Skills and Competencies
- In-depth knowledge of nursing theory and practice within a patient-centered care framework.
- Strong understanding of evidence-based practices and the ability to apply this knowledge effectively.
- Proficient in assessing patient needs and developing comprehensive care plans.
- Excellent leadership and communication skills, with the ability to foster collaborative relationships with patients, families, and healthcare professionals.
Duties
- Lead care coordination efforts by engaging with the interprofessional team to establish discharge goals and rehabilitation needs.
- Monitor patient progress and adjust care plans as necessary to ensure quality and efficiency.
- Act as a resource for both staff and patients regarding care management and discharge planning.
- Advocate for patients' rights and promote their autonomy and dignity throughout the care process.
