About the Opportunity

The Nova Scotia Health Authority is seeking a skilled and compassionate Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in assessing, planning, and coordinating care for patients and their families.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop personalized care plans
  • Collaborate with healthcare teams to ensure seamless transitions of care
  • Facilitate access to community resources and services to support patients' independence
  • Provide education and support to patients and their families to promote self-management and empowerment
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in dedicated case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and collaboratively in a busy multidisciplinary environment
  • Ability to multi-task, prioritize workload, and manage time efficiently
