About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in assessing, planning, and coordinating care for clients referred to Continuing Care. Your expertise will help individuals and families navigate complex health situations and access the resources they need to thrive in their communities.

Key Responsibilities:

  • Assess client needs and develop personalized care plans
  • Authorize services and make referrals to community resources
  • Collaborate with interprofessional teams to facilitate hospital discharge and community reintegration
  • Provide ongoing case management and support to clients and their care partners

Requirements:

  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long term care setting
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Valid driver's license and access to a reliable vehicle

Hours and Compensation

This is a permanent, full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The hourly rate is $46.75. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
