Job Summary
Southbridge Care Homes is seeking a skilled Registered Practical Nurse to join our team at West Park Health Centre, a 69-bed long-term care facility in Ontario. As a Registered Practical Nurse, you will play a vital role in enhancing the effective management and quality of nursing practice, ensuring the highest standard of care for our residents.
Key Responsibilities
- Provide resident care in accordance with established nursing standards, principles, and administrative policies, observing, reporting, and recording vital signs, symptoms, and conditions of residents, and initiating remedial health measures as necessary.
- Assess residents and develop, implement, review, and evaluate resident care plans, monitoring residents' health, noting changes in condition, and assessing the need for referral to doctors, processing and implementing physician orders.
- Organize and direct functions and activities related to the provision of care to residents, including restorative care and palliative care as required, interpreting and implementing established policies, procedures, and regulations.
- Plan, direct, supervise, and evaluate the work of personal care staff assigned to the unit, orienting and training new personnel, and participating in in-service education, monitoring performance, and conducting performance appraisals, referring problems to the Nursing Lead.
- Ensure safe and accurate medication administration to 35 residents.
Qualifications
- Current certificate of competence from the College of Nurses of Ontario required.
- Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
- Comprehensive knowledge of nursing and healthcare practices, as well as knowledge in Infection Control Practices, IPAC certification preferred.
- Experience interacting effectively with persons who have cognitive impairments, using approaches like G.P.A and P.I.E.C.E.S.
- Previous experience working in Long Term Care in Ontario, knowledge and experience meeting the standards of care, set forth in the relevant policies and procedures, and the ability to uphold the same.
- Effective human relations skills and demonstrated commitment to working with seniors.
- Effective communication skills and ability to work independently and collaboratively.
- Strong leadership skills and participation in the change of work flow process.
- Familiarity with P.C.C. and e.M.A.R.
- Current First Aid and C.P.R. certification preferred.
