We are seeking a skilled Registered Nurse to join our team at Journey Home Hospice, a unique and innovative program operated by Saint Elizabeth Foundation. As a Registered Nurse, you will play a vital role in providing high-quality care to patients experiencing homelessness and structural vulnerability.
Key Responsibilities- Provide compassionate and person-centered care to patients in a trauma-informed and culturally safe environment
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments and develop care plans that address the physical, emotional, and social needs of patients
- Work closely with patients, families, and caregivers to ensure seamless transitions and continuity of care
- Participate in ongoing education and training to enhance skills and knowledge in palliative care and trauma-informed care
- Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse (RN)
- LEAP Certificate Required / Willing to complete LEAP Certificate within 6 months of hire
- Minimum 1 year Hospice Palliative care experience and ongoing continuing education is preferred
- Experience working with vulnerable populations
- Mental Health Nursing Certification or equivalent training preferred
