We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our team at Journey Home Hospice, operated by Saint Elizabeth Foundation.

About the Role

This is a unique opportunity to make a meaningful difference in the lives of patients experiencing homelessness and structural vulnerability. Our 13-bed facility in downtown Toronto and four beds in Windsor provide a specialized trauma-informed, culturally safe, and harm-reducing approach to care.

Responsibilities
  • Deliver high-quality patient care with compassion, empathy, and understanding
  • Collaborate with an integrated healthcare team, including PSWs, RNs, physicians, nurses, and hospice volunteers
  • Maintain accurate records and communicate effectively with colleagues and management
Requirements
  • Minimum 1 year experience in Hospice Palliative care; ongoing continuing education preferred
  • Mental Health Nursing Certification or equivalent training preferred
  • Ability to work independently and as part of a team; effective problem-solving and conflict management skills
  • A current First Aid and CPR certificate; availability to work 12-hour shifts
