Job Overview

As a Registered Nurse at Halton Healthcare, you will be responsible for delivering specialized patient care within a dynamic complex continuing care unit. Your focus will be on supporting patients who require medically intricate care, gradual rehabilitative services, reactivation, or palliative interventions. Additionally, you will provide essential support to the families of these patients.

Key Responsibilities

  • Coordinate patient care, ensuring a smooth transition through the continuum of healthcare services.
  • Educate patients and their families on care processes and health management.
  • Administer medications and perform procedures in accordance with established protocols and guidelines.
  • Conduct comprehensive health assessments and meticulously document any observations or changes in patient conditions.
  • Collaborate with multidisciplinary health teams to revise individualized care plans aimed at achieving optimal patient outcomes.
  • In the role of RN, you may also take on charge responsibilities, providing guidance and support to fellow nursing staff.

Required Qualifications

  • Active registration in good standing with the College of Nurses of Ontario.
  • Current certification in Basic Life Support (BLS) approved by Heart & Stroke.
  • Proven experience in managing patients with complex needs, including tracheostomies, enteral/parenteral nutrition, CADD pumps, PICC lines, IV therapy, wound care, and fecal management systems.
  • Completion of a physical health assessment course.
  • Minimum of one year of nursing experience in a Long Term Care environment.
  • Exceptional communication skills, both verbal and written.
  • Demonstrated compassion, patience, and empathy in patient interactions.
  • Proven leadership capabilities.
  • Ability to prioritize and manage multiple tasks in response to the evolving needs of patients.
  • Proficient in working collaboratively within a team setting, while also being capable of independent work when necessary.
  • Familiarity with computer systems and CCRS data entry is an asset.
  • Commitment to ongoing professional development and education.
  • Strong organizational and time management skills.
  • Ability to meet the physical demands of the nursing role.
  • A satisfactory record of employment and attendance.

Work Schedule : Rotating shifts, including weekends, with various 12-hour shifts.

Compensation : Competitive hourly wage.
