We are seeking an experienced Occupational Therapist to join our team at Home and Community Care Support Services. As an Occupational Therapist, you will play a vital role in providing patient-centered care to individuals in their homes and communities.
Job Summary:The successful candidate will be responsible for assessing patients' needs, developing and implementing treatment plans, and collaborating with healthcare professionals to ensure seamless care delivery. Key Responsibilities:- Conduct comprehensive assessments of patients' physical, emotional, and cognitive abilities to identify areas for improvement.
- Develop and implement individualized treatment plans to enhance patients' functional abilities and independence.
- Collaborate with healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care delivery.
- Provide education and support to patients and their families to promote healthy behaviors and self-care.
- Document patient progress and outcomes in accordance with regulatory requirements and organizational policies.
- Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
- Minimum 2 years of experience in a community or healthcare setting.
- Strong communication and interpersonal skills to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team to achieve patient-centered outcomes.
- Valid driver's license and access to a vehicle.
