About the Opportunity

We are seeking a skilled Health Care Social Worker to join our team at Nova Scotia Health and IWK Health. As a member of our multidisciplinary team, you will provide a range of interventions to clients and their families, enabling them to achieve their optimal level of functioning and health.

Key responsibilities include:

  • Conducting psychosocial assessments and providing adjustment therapy, supportive therapy, crisis counseling, and resource counseling
  • Developing and implementing discharge plans and providing case management services
  • Advocating for individuals and groups within the institution and community
  • Collaborating with other healthcare professionals to provide comprehensive care

We are looking for a candidate with a Master's Degree in Social Work from a recognized university and registration with the NSCSW. A minimum of 2 years of clinical experience as a clinical Social Worker is required. Knowledge of the fundamentals of human behavior, including systems theory, family dynamics, group dynamics, and theories of behavior change, is essential.

Additional requirements include:

  • Awareness of local community resources and agencies
  • Valid Nova Scotia Driver's License and access to a reliable automobile
  • Experience working with patients with complex chronic health conditions
  • Competencies in other languages, with French being an asset
