At Banwell Gardens Care Centre, we are seeking a highly skilled and experienced Lead Licensed Practical Nurse to join our team as a Quality Improvement Specialist. This is a unique opportunity to make a meaningful impact on the quality of care we provide to our residents.
Key Responsibilities:- Lead the internal Quality Improvement program and ensure that all quality management and risk management related activities are reviewed, discussed, and follow-up actions identified as necessary.
- Collaborate with the Leadership Team to develop and submit the Annual HQO Quality Improvement plan and Progress report for the Home.
- Train new staff on the RQI process and the Resident/Family and Employee Surveys interview questions and surveys.
- Utilize the Health Quality Ontario website for resources to advance quality improvement activities.
- Provide coaching, mentoring, and facilitating teams on the use of lean models for quality improvement projects and have knowledge on the use of various QI tools such as fishbone diagram, PDSA, flow charts, etc.
- Monitor quality indicators on the public website of CIHI and Your Health System.
- Facilitate quality meetings and have the ability to present and communicate data analysis, trends, and Quality Improvement activities to interdisciplinary team members, stakeholders, and Residents' and Family Council.
- Registered health professional licensed under the Health Disciplines Act registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60.
- Comprehensive knowledge of health care practices, trends, and issues in long-term care and impacts on other health care sectors/community at large.
- Proven management experience in LTC.
- Excellent knowledge of Quality/Risk Management philosophies and programs.
- Master trained in Resident/Family and Employee Surveys/Resident Quality Inspection process.
- Working knowledge of relevant computer applications and applicable legislation.
- Strong interpersonal and team building skills.
- Ability to identify training needs of the team and individuals.
