Job Summary

We are seeking a highly skilled and compassionate Pharmacist to join our team at one of our long-term care facilities. As a Pharmacist in this role, you will be responsible for providing safe and effective drug therapy to our residents, while also building strong relationships with our nursing staff and other healthcare professionals.

Responsibilities

  • Provide safe and effective drug therapy to residents, including annual interdisciplinary care conferences and quarterly medication reviews
  • Lead offsite vaccination clinics and consult with nursing staff to build drug therapy regimens for complex disease states
  • Answer questions from residents and families related to drug therapy and provide education seminars to nursing staff on various health topics
  • Participate in annual medication reviews (SMAPs and mental health medication reviews)

Qualifications and Skills

  • Experience in long-term care, geriatric, or palliative pharmacy practice
  • Excellent communication and organizational skills, with a strong attention to detail
  • Ability to work independently and collaboratively as part of a team
  • Strong problem-solving and collaboration skills
