COMMUNITY SUPPORT WORKER

Position Overview
As a Community Support Worker, you will be an integral part of a dedicated team, providing essential care and assistance to individuals residing in community-based homes. Your role will involve fostering independence and self-determination among those you support, enabling them to lead fulfilling lives while upholding high standards of care, health, and safety. Your compassionate nature and ability to collaborate effectively within a multidisciplinary team will significantly contribute to the success of your colleagues and the individuals you serve.

Work Schedule: Full-time, 40 hours per week, from 3 PM to 11 PM, Thursday through Monday.

Key Responsibilities

  1. Respect and advocate for the rights and choices of supported individuals.
  2. Engage effectively with the individuals receiving support.
  3. Empower individuals to become active participants in their community.
  4. Contribute to the creation, execution, and documentation of Personal Service Plans.
  5. Build positive relationships with family members, professionals, coworkers, and community members.
  6. Assist with daily living activities and medication management.
  7. Demonstrate a solid understanding of behavior management techniques, including mental health considerations.
  8. Document daily activities and personal health information (PHI) in our case management system.
  9. Collaborate with supported individuals to maintain a clean and well-organized home, including food preparation and light housekeeping tasks.
Qualifications
  1. Completion of Grade 12 education.
  2. Community Support Worker Diploma or equivalent qualification.
  3. A minimum of 2 years of experience in community social services or healthcare.
  4. Current Emergency First Aid/CPR and Food Safe certifications.
  5. Prior training in behavior management.
  6. Strong verbal and written communication skills with team members, professionals, families, and community members.
  7. Non-verbal communication proficiency.
  8. Valid Class 5 driver's license.
  9. Acceptable Driver's Abstract.
  10. Criminal record clearance for working with children and vulnerable adults.
  11. TB Test clearance.
  12. Moderate proficiency in computer skills.
  13. Eligibility to work in Canada.
