About the Role

Pacific Coast Community Resources is seeking a skilled Community Support Worker to join our team. As a Community Support Worker, you will play a vital role in providing care and support to individuals living in staffed homes within their community.

Key Responsibilities:

  1. Ensure the rights and choices of the individuals we support are respected and advocate as needed.
  2. Develop and maintain effective relationships with individuals, families, and community members.
  3. Participate in the development, implementation, and documentation of Personal Service Plans.
  4. Assist with activities of daily living and medication administration.
  5. Record daily activity and personal health information (PHI) of the individuals we support on our case management database.
  6. Work collaboratively with the individuals we support to ensure their home is clean and well maintained at all times.

Requirements:

  1. Grade 12 education.
  2. Community Support Worker Diploma or equivalent.
  3. Minimum 2 years of experience in community social services/health care.
  4. Current Emergency First Aid/CPR and Food Safe certificates.
  5. Previous behaviour management course/training.
  6. Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
  7. Non-verbal communication skills.
  8. Class 5 driver's licence.
  9. Satisfactory Driver's Abstract.
  10. Criminal record clearance for children and vulnerable adults.
  11. Proof of COVID-19 vaccination status.
  12. TB Test.
  13. Moderate computer literacy.
  14. Eligibility to work in Canada.
