Qualifications and Requirements

We are seeking a highly skilled and experienced Social Worker to join our Primary Care Network team at Island Health. The successful candidate will possess a Bachelor's Degree in Social Work from an accredited university and be registered with the British Columbia College of Social Workers.

Key qualifications include:

  • Two years' recent experience in a healthcare setting working with medically complex clients with concurrent disorders.
  • Comprehensive knowledge of chronic health challenges, geriatrics, community health principles, human development, behavior, and family/group dynamics.
  • Working knowledge of applicable legislation and available programs and services.

The ideal candidate will also possess excellent counseling skills, including crisis, loss, and grief counseling, as well as the ability to facilitate and build effective relationships with primary care providers, other disciplines, and community partners.

Key Responsibilities

The successful candidate will be responsible for:

  • Providing counseling services to patients and their families.
  • Facilitating and building effective relationships with primary care providers and community partners.
  • Developing and implementing care plans that address the unique needs of patients.
  • Collaborating with other healthcare professionals to provide comprehensive care.
