Lifemark Health Group is seeking a skilled Physician to join our interdisciplinary team in providing comprehensive rehabilitation services to Canadian Veterans. As a key member of our team, you will work closely with our clinicians to assess and intervene in the management of complex medical conditions.
Key Responsibilities- Conduct thorough assessments of presenting eligible health problems
- Perform medication reviews and identify drug therapy problems
- Clarify relevant medical issues, including comorbid conditions
- Clarify diagnosis and prognosis related to eligible health problems
- Identify conditions that may hinder progress in rehabilitation
- Collaborate with the interdisciplinary team on treatment recommendations
- Communicate with Veterans, support persons, and family members to promote understanding of medical conditions and recommendations
- Participate in de-brief discussions with the interdisciplinary team
- Engage in targeted follow-up care through a consultative model
- Collaborate with PCVRS Medical Director(s) and engage in continuous quality improvement
- Doctor of Medicine and valid license to practice in respective field in province/territory as applicable
- Experience in the management of complex medical conditions
- Excellent verbal and written communication skills
- Experience working with a variety of health professionals and disciplines in an interdisciplinary team environment
