Job SummaryWe are seeking a highly skilled Physiotherapist to join our dynamic Rehabilitation Team. As a Physiotherapist with CarePartners, you will play a critical role in providing high-quality patient care in the community and in patients' homes.
Key Responsibilities- Conduct professional assessments of patients in the community and in their homes
- Develop and maintain effective care plans in collaboration with the Home and Community Care Support Services (HCCSS)
- Work with patients to set and achieve time-specific, measurable goals and outcomes
- Provide one-on-one therapeutic support to clients
- Promote independence of clients in the home and in the community
- Master's Degree (or equivalent) in Physiotherapy from a recognized university
- Registration with the College of Physiotherapists of Ontario
- Proven computer skills, including email, word processing, and work scheduling systems
- Strong assessment skills and ability to participate in effective treatment programs
- Excellent English verbal and written communication skills
- French language skills are an asset
