Community Health Nurse

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Home and Community Care Support Services. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes and communities.
Key Responsibilities- Provide advanced nursing care to patients with complex medical needs, including palliative care and chronic disease management.
- Collaborate with healthcare professionals, community service providers, and patients to develop and implement individualized care plans.
- Conduct home visits to assess patient needs and provide education and support to patients and their families.
- Participate in interprofessional teams to coordinate care and ensure seamless transitions between healthcare settings.
- Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class (RN EC).
- CHPCN certification or commitment to obtain.
- Working knowledge of community resources and roles of healthcare professionals.
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
- Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment.
- Specialized education in palliative pain and symptom management.
- LEAP certification.
- Palliative/end-of-life care experience.
- Awareness of Medical Assistance in Dying Legislation.
- Complex chronic disease management and psychogeriatric care experience.
