About the Role:

We are seeking a skilled and compassionate Registered Practical Nurse to join our team as a Community Care Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Participate in scheduled visits in the community to provide patient-centered care and support
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy living and disease management
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Experience working with vulnerable populations
  • Passion for making a difference in your community
