Job Title: Family Practitioner - Sooke Outreach Services

We are seeking a part-time Family Practitioner to join our team at the Sooke Food Bank and Sooke Shelter in Sooke, B.C. This role will provide outreach services to adults and children impacted by poverty, colonization, stigma, and homelessness.

Key Responsibilities:
  • Deliver primary episodic care services that address immediate medical needs and work towards decreasing barriers and social determinants of health in accessing care.
  • Provide OAT and primary care services to a broad range of clients, supported by a primary care RN, an MHSU RN, and a MOA.
Requirements:
  • Family Medicine certification and licensure in British Columbia.
  • Experience working with vulnerable populations and providing outreach services.
  • Strong communication and interpersonal skills.
