As a Client Care Coordinator within BC Mental Health & Substance Use Services, you will play a pivotal role in enhancing the quality of care for clients in correctional facilities. This position is integral to ensuring that individuals receive comprehensive healthcare services in a supportive environment.
Key Responsibilities
- Lead and coordinate client care initiatives, implementing innovative strategies to enhance service delivery. Collaborate with interdisciplinary teams to establish individualized care plans that address the diverse needs of clients.
- Support staff in adhering to care standards and clinical pathways, ensuring compliance with organizational policies and quality assurance protocols. Facilitate effective communication among team members to address operational challenges and maintain continuity of care.
- Participate in client evaluations and admission processes, contributing insights on care effectiveness and administrative decisions. Conduct thorough assessments to determine priority care requirements.
- Monitor client progress and provide consultation to healthcare team members. Foster interdisciplinary collaboration to address treatment concerns and ensure effective crisis management.
- Build and maintain therapeutic relationships with clients, engaging them in their care planning and supporting their self-management efforts.
Qualifications
- Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
- Completion of relevant specialty nursing programs and a minimum of three years of recent experience in a related clinical setting.
- Possession of a valid driver's license and access to a vehicle for local travel may be necessary.
- Successful completion of Occupational First Aid II or equivalent is preferred.
Skills and Expertise
- In-depth knowledge of primary care practices, mental health conditions, and substance use disorders.
- Proficiency in evidence-based nursing practices, including pharmacology and therapeutic interventions.
- Ability to provide effective leadership and clinical guidance to team members.
- Strong communication skills for collaboration with clients and healthcare professionals.
- Commitment to understanding and implementing principles of reconciliation and cultural safety.
