Primary Care Network Manager

We are seeking an experienced Primary Care Network Manager to join our team at EightSix Network. As a key member of our organization, you will play a critical role in developing and implementing primary care networks and patient medical homes on the North Shore.
Job Summary:The Primary Care Network Manager will be responsible for managing and coordinating the development of primary care networks and patient medical homes, ensuring alignment with the North Shore's PCN strategic objectives and Ministry of Health policy directives. This role will involve engaging with family physicians, medical office staff, and other stakeholders to advance the development and implementation of patient medical homes and primary care networks.
Key Responsibilities:- Develop and implement plans to establish primary care networks and patient medical homes, ensuring successful completion of activities.
- Establish relationships with physician practices and clinics to support their transition to patient medical homes, providing interdisciplinary team-based care and other PMH attributes.
- Oversee the placement and orientation of nurses and allied health into patient medical homes, working with teams to successfully implement interdisciplinary care.
- Participate in the recruitment of VCH clinical staff for patient medical homes, providing leadership, guidance, coaching, and support to staff in partnership with the VCH manager for primary care.
- Commit to the mission of effective care coordination for indigenous patients wishing to access medical care and indigenous cultural healing.
- Work with the First Nations Aboriginal Primary Care Network (FNAPCN) as needed to promote coordination of aboriginal services and remain aware of current indigenous services offered by the FNAPCN.
- Track progress of the development of patient medical homes within the PCN, monitoring and reporting on the status of implementation plans, including issues and obstacles.
- A Master's Degree in a relevant discipline with minimum five years' progressive leadership experience, including experience managing and/or supervising interdisciplinary healthcare programs and/or health care projects.
- Excellent communication, consultation, and relationship building skills to function within a complex interdisciplinary environment, including demonstrated experience working effectively with physicians.
- Maintains a broad knowledge of provincial health care delivery systems, with a focus on primary care in BC and/or Alberta.
- Demonstrates comprehensive knowledge of project, program, and change management principles and methodologies, and ability to coach team members on these skills.
This is a hybrid role, with 3 days per week in-office. The successful candidate will be required to work Monday – Friday during business hours 9 AM – 5 PM, with some flexibility to accommodate evening and early morning meetings. Access to a vehicle for local travel is required.
We offer a competitive salary range of $90,000 - $120,000 per year, depending on experience. Qualified applicants please submit a formal cover letter and resume.
