  • Oversee case management and service coordination
  • Assist in planning for transitions and discharges
  • Participate in preventive and promotional health initiatives

Required Qualifications:

  • Bachelor's degree in Social Work
  • Active registration with the Alberta College of Social Workers (ACSW)

Preferred Qualifications:

  • Experience in managing complex care scenarios
  • Demonstrated advocacy skills within the healthcare system
