We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Pro Vita Care Management. As a LPN, you will play a vital role in providing high-quality care to our residents in Long Term Care in Kamloops.
Job Summary:
The successful candidate will be responsible for observing, monitoring, and documenting resident physical and psychological conditions, as well as providing personal care and support as required. You will also be expected to ensure adherence to Infection Control Practices and provide education as needed.
Responsibilities:
- Assess resident health status, including differentiating between normal and abnormal physical findings, recognizing deviations from mental health baseline, and differentiating between dementia and delirium.
- Monitor residents' actual and potential strengths and limitations, including physiological, psychological, socio-cultural, and spiritual needs.
- Take appropriate action, such as consulting with the primary physician or making referrals to the Mental Health Team.
Requirements:
- Graduation from a recognized Practical Nurses program or equivalent combination of education, training, and experience recognized by the BCCNM.
- Holds an active registration with the BCCNM as an LPN.
- Maintain Occupational First Aide Level 1 status.
- Passes and complies with Ministry of Justice Criminal record Check upon hire and every 5 years thereafter.
- Ability to effectively communicate in English, both verbal and written.
Preferred Qualifications:
- Gerontological experience preferred.
- MDS education (Gold Care or Point Click Care).
