Nurse Practitioner

Halton Healthcare is seeking a highly skilled Nurse Practitioner to join our Addiction Services team. As a key member of our inter-professional team, you will provide clinical expertise and leadership in the assessment, treatment, and support of individuals with Mental Health & Substance Use Disorders.
Key Responsibilities- Conduct comprehensive bio-psychosocial assessments and develop person-centered treatment plans
- Provide specialized assessment, coordination, and treatment for individuals living with Mental Health & Substance Use disorders
- Collaborate with inter-professional teams to establish and maintain partnerships with community organizations
- Develop and implement evidence-based models of treatment intervention and their application to diverse populations
- Provide opioid addiction treatment with a harm reduction approach to care
- Link clients to primary care and foster existing relationships with primary care to ensure client needs are being met
- Master's Degree Program in Nursing or recognized equivalent
- Current registration in the Extended Class with the College of Nurses of Ontario
- Nurse Practitioner's Certificate from a recognized program
- Minimum three years' experience working in the field of substance use disorders/addiction
- Minimum five years of experience working in the field of Mental Health including both inpatient and community-based services
