We are seeking a highly skilled and compassionate Cancer Nurse Navigator to join our team at the Government of Northwest Territories. As a key member of our interdisciplinary team, you will play a vital role in supporting enhanced access to cancer support services for clients and families.
Key Responsibilities- Act as a single point of contact for clients and families during key transition points along the cancer care continuum, including initial diagnosis, active treatment, and end-of-treatment phases.
- Collaborate with Territorial, Regional, and Community Health Centre sites to provide coordinated and integrated care for clients.
- Engage in direct cancer patient care during inpatient and outpatient departments at Stanton Territorial Hospital.
- Provide and educate clients on survivor care plans, promoting a holistic and comprehensive approach to care.
- Act as an advocate, facilitating communication between clients, their families, healthcare professionals, and other stakeholders.
- Baccalaureate Degree in Nursing and completion of an oncology nursing program.
- Minimum of 5 years nursing experience, with a minimum of 2 years of oncology nursing experience preferred.
