Cancer Care Coordinator

Join our team as a Cancer Nurse Navigator and play a vital role in supporting enhanced access to cancer support services for patients and families in the Northwest Territories.
About the RoleThis is an exciting opportunity to work collaboratively with healthcare professionals, patients, and families to provide a single point of contact for coordination and integration of services across the cancer care continuum.
As a Cancer Nurse Navigator, you will engage in the coordination of safe, integrated, comprehensive, and individualized care for patients with cancer, providing education and support to help them navigate the cancer care system.
You will work closely with healthcare teams to identify and address the unique needs of each patient, promoting a holistic and comprehensive approach to care.
Key Responsibilities- Coordinate and integrate services across the cancer care continuum, including initial diagnosis, active treatment, end-of-treatment, and survivorship care.
- Provide education and support to patients and families to help them navigate the cancer care system.
- Work collaboratively with healthcare teams to identify and address the unique needs of each patient.
- Engage in direct cancer patient care during various inpatient and outpatient departments at Stanton Territorial Hospital.
- Respect patient confidentiality and maintain accurate records.
To be successful in this role, you will require:
- A Baccalaureate Degree in Nursing and completion of an oncology nursing program.
- Minimum of 5 years nursing experience, with a minimum of 2 years oncology nursing experience preferred.
