Licensed Practical Nurse Team Leader

Job Summary:
The Licensed Practical Nurse Team Leader is responsible for coordinating Home Support Services by supervising a team of Licensed Practical Nurses, Community Health Workers, and other unregulated members. This role involves overseeing the implementation of client care plans, providing training and evaluation of care provided by staff, and advocating for changes that improve client care.
Key Responsibilities:
- Collaborate with the interdisciplinary team to perform assessments, make nursing diagnoses, and implement personal and nursing care for clients in a community health setting.
- Operate within the competency guidelines and full scope of practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to organizational policies, procedures, standards, and unit-specific protocols.
- Provide leadership and guidance to the team, ensuring that client care plans are implemented effectively and that staff are trained and evaluated accordingly.
